How about an add-on sales extension?
For example if I purchase a new shirt I get a pop up screen during the checkout process asking if I would like to purchase a new tie. I then have the option of adding it to my basket or continuing on without buying. Of course if the tie was already in my basket I wouldn't get the popup for a tie but something else (set by the store owner). You should also have the option of offering a discount on the suggested item.
Miva has something like this and I found it very effective resulting in lots of add-on sales.
