I had a order go though the gateway (authorize.net) and was approved on the CC but it make no order in the system but pass a order number to the gateway (authorize.net). I have the customer in the customer table but no order in the order table. Any Ideas what happen?

In the Online Setup do you have the keep cancelled orders set to No?

If so possibly change this to Yes so the roders are not deleted from the system.

What may have happened is the customer hit the back button after making the order and clicked cancel for some reason.

Without further info though it is hard to say what culd have caused this.
