As far as I can tell the Upgrade Manual does not have instructions for upgrading the header and footer template to 7.00 from 6.00. I only see instructions for 5.5 and 6.50. Any advice?

I think 6.00 would be the same with 6.50 as the header and footer in 6.00 have identical format with 6.50.
